2024 JUNIOR POLO TOURNAMENT ENTRY FORM

Teams will be arranged by the HPC Junior Committee.

Conditions of Tournament:
  • Players must provide their own horses, tack and equipment. Required equipment includes boots with heels and helmet with goggles or facemask.
  • Player entry fee must be paid in full by Friday, May 17, 2024
  • Players in the Advanced (NYTS) Level ONLY must hold a current USPA handicap. www.uspolo.org
  • Advanced (NYTS) Level Players are required to wear a helmet that passes the NOCSAE ND050 Standard Performance Specification
  • A current HPC release form must be executed by all players/parents prior to participating in the tournament. Click here for Release Form

Submitted forms are sent to the HPC office
